**About TCP & ACAS**

TCP aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department. The program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and finalise and access their longer-term care arrangements or maintenance at home

ACAS Program provides comprehensive, holistic assessments as part of My Aged Care for frail older persons or younger persons with a disability. The assessment considers the physical, medical, psychological and social needs of the client, an evaluation of their functional potential and develops care plans that promotes restoration of function and maximises independence. The ACAS program links people in with Commonwealth Home Support Program, Private services and delegable Commonwealth funded services such as residential respite, permanent care, Home care packages and Flexible care options.

**About The Role**
- Predominently based at Kingston Centre, with travel to Dandenong North and Cranbourne on regular basis.

The Manager of TCP/ACAS will work closely with the Director Clinical Operations and Senior clinicians to provide clinical leadership, risk management and service development to a large multi-disciplinary team. Innovation, continual improvement and actively seeking research opportunities are core priorities in order to improve outcomes for our community.

**About You**

You have established management/significant leadership skills within healthcare and are committed to improving outcomes for older Australians.

**What You Need**
- Relevant Qualification (Allied Health or Nursing)
- Demonstrated experience in leadership roles
- Current AHPRA registration (if applicable to your profession)
- Australian drivers license

- Post graduatue qualifications (clinical or management) - preferrable
