The assessment considers the physical, medical, psychological and social needs of the client, an evaluation of their functional potential and develops care plans that promotes restoration of function and maximises independence. The ACAS program links people in with Commonwealth Home Support Program, Private services and delegable Commonwealth funded services such as residential respite, permanent care, Home care packages and Flexible care options.
