**What You Will Be Doing**

The Transition Care Program is seeking an experienced, innovative and flexible Clinician to work as a member of the TCP team.
The program is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine their long term care arrangements.

**What You Need**

The successful applicant will need to be flexible in terms of working across both residential and community areas. The Case Manager will need to demonstrate sound clinical assessment skills, and to have an understanding of the issues affecting patients with complex social and accommodation needs and an interest in aged care issues.
The successful applicant will need to be highly motivated, and enthusiastic and to be able to work both independently and as part of a team.
