**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.
- The primary location for the role is the StaffHub at North Geelong, however may be moved to other Barwon Health locations depending on service delivery needs.

**About the role**
- The Connected Care Diabetes Educator will contribute to, and be part of the community-based, interprofessional team to support consumers and carers to achieve an optimum level of health independence through:

- Provision of diabetes education and self-management skills to clients diagnosed with diabetes. This includes individual assessment, education and shared care planning and the provision of ongoing support and advocacy.
- Provision of comprehensive, co-ordinated and integrated multidisciplinary services.
- Provision of consumer-centred care.
- Contribution to the development and delivery of high-quality, cost-effective community-based Diabetes Education services.**:

- Contribution to meeting relevant performance indicators.
- The role may include components of care coordination as part of the Chronic and Complex Service multidisciplinary team.

The team provide comprehensive assessments, care coordination, linkages to services and some specialist allied health and medical care. The model of care is responsive and agile to client and organisational needs. The team includes care-coordinators, nurses, medical staff and allied health professionals including psychology, diabetes educator, physiotherapist, and social workers.
