
Disability Sil House Leader

_**Your main responsibility is to be the main liaison between on the ground shifts & clients/ SIL Houses, and the Service Delivery Manager, who will be office based.**_
- **At times, your roster may need to be changed or adapted to suit the companies and client’s needs. It is expected that as the House Leader you will make all attempts to try and assist where required to assist the business in these instances.**_
**Your key responsibilities will be**:
**Medication Management**:
- Ensuring Clients medication is always a fortnight in advance, so there is never a chance for a client to run out of medication due to you missing a shift, or public holidays or the like.
- Liaising with Staff if you are unable to do a pickup yourself to pick up medication from chemist.
- Liaise with staff if scripts are running out, and a doctor’s appointment needs to be made to get updated scripts. Please always ensure that a doctor appointment form is always taken to this appointment and these templates are always available in each house.
- Reviewing the medication charts each week and seeing if there are any errors on medication charts. Contacting staff to ask why the mistake was made, and advising SDM if you notice any errors on the medication charts for them to not down from an operation level.
- ALWAYS ensuring there are enough medication chart templates.
- Collating the medication charts for SDM to pick up weekly.
**Client Weekly Grocery Shopping**:
- Ordering weekly groceries for all clients
- Ordering tea and Coffee for houses
- Basic household cleaning products/supplies
- Checking the fridge and making sure it is always clean, and that staff are using all food purchased and that old food is thrown out.
- Monitoring the client’s food intake and ensuring that staff are cooking meals, and not just taking the ‘easy way out’ in terms of cooking for the client.
- Discussing with SDM if you have any concerns about staff not managing food intake, cooking proper meals for clients.
**SIL Household monitoring/maintenance**:
- Monitoring the household maintenance
- Are there any issues with the property maintenance - e.g., leaking taps, insects smell etc.
- Ensuring all staff are contributing to the cleaning chart and are signing this off.
- Ensuring clients are doing their chores, and this is being signed off on the chore chart (obviously if they are unable to perform this task, they are exempt)
**SIL Client Appointment Management**:
- Managing the Online diary and the hard copy diary for all client’s appointments - advising SDM of all appointments once confirmed and the purpose of the appointment.
- Ensuring that the medical appointment form is being filled out for each appointment and collating those each week for the SDM to collect.
- Collecting and reviewing any discharge summaries.
- Liaising with staff members who may be on shift for certain appointments and discussing days and times of appointments where necessary. If any changes need to be made to rosters, please liaise with SDM on this to get them to approve and make any changes in Brevity.
**Staff Training & Management**:
**You are responsible for in house training, and staff buddy shifts for all new workers.**
- Conducting all in house training to new staff members
- This involves you doing in house training at each property, taking them through all documentation, process, and procedures.
- Briefing and ensuring staff are full across all clients care plans - test them on questions to ensure their knowledge.
- Performing buddy shifts
- Ensuring new staff all sign the training documentation form - please collate this in the documentation for SDM to collect each week.
- Providing feedback to SDM post all training & buddy shifts to advise on competence, comprehension, and any additional feedback.
**Job Type**: Casual
Pay: $43.00 per hour
Expected hours: 24 - 38 per week
