**About the role**

In this role you will assist people with a disability to realise their home and living goals by supporting them on a journey to their new home with Sunnyfield. You will provide detailed information on the new home and living vacancy listings to Sunnyfield’s Customer and Market Engagement Team in order to attract new inbound leads. This is a Full Time role.

Your key responsibilities will be:

- Provide detailed information on the home and living vacancy listings in order to attract new inbound leads.
- Maintain CRM records of prospective leads for Sunnyfield home and living services
- Coordinate service visits and transition planning meetings
- Assess, qualify and recommend prospective leads for placement into Sunnyfield’s home and living services, involving client compatibility matching, support needs assessment, risk analysis and financial modelling.
- Reporting on occupancy and vacancy rates as well as lead progression

**About you**
- Minimum 2 years’ experience in a similar role
- Highly developed communication, negotiation and problem-solving skills
- Extensive experience in case management, including referral, needs assessments, support planning, risk assessments and needs assessment reviews, with people with a disability
- Excellent administrative and reporting skills, including the use of MS Office programs
- Strong interpersonal and relationship-building skills
- Motivated and resilient attitude towards goals
- Experience with working with people from diverse background
