Right at Home provide exceptional in-home care and support for a range of clients in the **Perth South-Eastern Suburbs and our Mission is to improve the quality of life for those we serve.**
Due to continued growth of our business, we urgently seek a dynamic individual to take on a newly created role of Relationship Officer responsible for raising awareness of our organisation across Perth and the South-Eastern suburbs of Western Australia, specifically, to grow the Perth region.
The Relationship Officer needs to demonstrate the ability to create digital marketing and promotional material across various social media platforms to raise awareness across the community and home care sectors and possess strong stakeholder management skills to build strong partnerships with these groups long-term.
You will report directly to an amazing Client Liaison Officer who is self-driven and committed to offering the highest level of service to clients we currently serve.
Your role will be to develop new business opportunities and you ideally should have:
- Experience in the Community or Home Care sector (advantageous but not essential)
- The ability to promote services and programs to external stakeholders across the health and human-services sectors.
We deliver services under a range of funding sources, including private pay clients and My Aged Care with our primary responsibility to provide quality home care and assistance to our fast-growing client base.
The successful applicant will be working with our care team, liaising with clients, family members, staff, and a wide range of stakeholders to ensure services are provided in accordance with client needs, and the goals and objectives of the business.
Additionally, completing assessments, reviewing client plans to ensure we deliver quality outcomes for all and to manage internal and external sales enquiries to develop and maintain new and existing referral sources.
**Qualifications**:
The Relationship Officer will be responsible for:
- Building strong connections with clients and their families
- Service coordination
- Organise support to clients with disabilities and monitoring and reporting on their progress
- Referring clients to agencies that can provide additional help
- Assessing client needs and resources for health, welfare and other facilities and services
- Maintaining accurate records of incidents and feedback as required
- Re-organising work priorities to meet the needs of a fast-paced environment
- Supporting all clients to meet their goals
- Conducting Client Care Assessments and Care Planning
- Supervision and management of Care Staff
- Discussing and promoting Right at Homes services with medical professions and referral sources
- Perth based with some travel
- Ability to work in a fast-paced environment
**Preferred Skills**:
- Experienced in assessing and planning client needs
- Sound understanding of the Aged Care and Disability systems is ideal
- A strong client focus, which is based on their goals, preferences and needs
- Demonstrated experience managing staff
- Current National Police Check
- Working with Children Check
- Full Drivers Licence
- Organisational, administrative and time management skills are essential
