**The Opportunity**
We’re currently seeking a compassionate, skilled, and motivated **Support Coordinator (COS)** to join our team. In this rewarding and dynamic role, you’ll be responsible for assisting NDIS participants to **navigate the NDIS system, build their independence, and coordinate the supports they need** to achieve their goals.
This role is perfect for someone who enjoys **problem-solving, working autonomously**, and building strong, person-centred relationships with participants and support networks.
**Your Key Responsibilities**
As a Support Coordinator, your work will be both strategic and hands-on. You will:
**Coordinate NDIS Supports**
- Assist participants in understanding their NDIS plans
- Help them identify, connect, and engage with suitable service providers (mainstream, community, and funded)
- Ensure services are aligned with their goals and funded supports
**Capacity Building & Advocacy**
- Work closely with participants to build their capacity for choice, control, and independence
- Provide coaching to help them understand how to use their NDIS funds and manage supports
- Advocate for participants when challenges arise with service access or delivery
**NDIS Plan Implementation**
- Develop and implement individual support plans based on the NDIS goals of each participant
- Monitor plan usage and support ongoing plan reviews and reassessments
- Liaise with the NDIA, Local Area Coordinators, and other stakeholders when required
**Community Engagement & Networking**
- Build and maintain strong working relationships with local providers and referral partners
- Promote inclusive practices and strengthen participants' connection to their local community
**Documentation & Compliance**
- Maintain accurate participant records, progress notes, and reports
- Ensure all work complies with NDIS Quality & Safeguarding standards
- Use case management systems to track outcomes and deliverables
**About You**
You are a compassionate, motivated individual who is committed to delivering high-quality support coordination. You bring a balance of professionalism, initiative, and empathy to everything you do.
**To succeed in this role, you will need**:
- Proven experience working with people with disability or in a support coordination role
- Solid understanding of the NDIS framework and its funding categories
- Ability to work both independently and collaboratively within a multidisciplinary team
- Excellent communication, interpersonal, and organisational skills
- Strong problem-solving ability and a capacity to manage competing priorities
- A person-centred, strengths-based approach to service delivery
- Confidence using digital platforms for case management and reporting
**Minimum Requirements**:
- Relevant qualifications in community services, disability, social work, or a related field
- Current Working with Children Check and NDIS Worker Screening Check
- National Police Clearance (or willingness to obtain)
- Current Driver’s Licence and reliable vehicle
- High level of computer literacy (e.g. MS Office, CRM systems)
