**About the role**
Based in Eight Mile Plains at our new offices, you will be part of a dedicated team with supportive management.

As a Client Liaison Officer, you will undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist them in achieving their identified goals. Support the client to manage and monitor their budget to ensure care packages are financially sustainable and monitor the services provided to ensure these are following relevant service models and guidelines. This role is based at our Logan Service.

**bout you**
- Certificate IV in Health, Community Services, Aged Care or Disability Support
- Proven understanding of assessments, packaged care and case management
- Ability to engage with clients working effectively with them to achieve their identified goals
- Provide support and guidance to clients with budget management and financial sustainability
- Exceptional communication skills
- Ability to advocate strongly for your clients
- A sound understanding of the community care system.
- A current National Police Clearance
- Current COVID-19 Vaccinations
- Qld drivers licence
