**About Us**:
Care Call Disability Services is a dedicated disability service provider offering support to individuals with disabilities. We deliver compassionate, culturally competent care to our clients, ensuring their well-being, independence, and social inclusion. We are currently seeking a Nepalese/Indian speaking Support Coordinator to join our team.

**Job Overview**:
As a Support Coordinator, you will be responsible for helping individuals with disabilities access essential services, create tailored support plans, and navigate complex systems to promote independence and well-being. Your role will involve advocacy, coordination of care, and working with multidisciplinary teams to ensure holistic support for clients. Fluency in Nepalese or Indian languages is required to assist our culturally diverse community.

**Key Responsibilities**:

- Conduct assessments of client needs and create individualized support plans in collaboration with clients and stakeholders.
- Connect clients with community resources, services, and supports, including healthcare, housing, and employment services.
- Act as an advocate, ensuring clients' rights are upheld and their voices are heard.
- Regularly monitor and review the effectiveness of support plans, adjusting as needed.
- Maintain accurate records of client interactions and progress.
- Collaborate with healthcare providers, social workers, and other community organizations.
- Educate clients and their families on available resources and strategies to achieve their goals.

**Pay**:
$40.00 - $60.00 per hour

**Requirements**:

- Fluency in Nepalese or Indian languages
- Previous experience as a Support Coordinator or Case Manager (minimum 1 year)
- Certificate IV in Disability, Community Services, or Mental Health (essential)
- Bachelor’s degree in Social Work, Psychology, Community Services, or a related field (desirable)
- Strong understanding of the NDIS and Care Plans
- NDIS Workers Screening Check and Working with Children Check (preferred)
- Australian Driver’s License and use of own car
- Strong problem-solving skills, punctuality, and trustworthiness
