Western Health is seeking talented and skilled Geriatricians to join their growing team, in both casual roles and for leave cover positions.

Leave cover roles include:

- Two 12 month part-time maternity leave cover positions - with estimated start dates in December and February.
- Two part-time long service leave cover positions, over December and January.

The Division of Aged, Cancer and Continuing Care provides quality health care for patients across Acute and Subacute Aged care, Rehabilitation services, Cancer Services, Palliative Care and Transition Care Services across four sites. Aged Care and Subacute Services play an integral role within the health care continuum, supporting patients to maximise their independence and functioning and, in doing so, minimise long-term health and community care needs. This diverse group of services is fundamental in promoting effective and seamless services across the care continuum. To ensure that patients have appropriate and timely access to these services, the Division also provides a Subacute and Non-acute Assessment and Pathways (SNAP) service to facilitate pathways to the right care setting.

The Geriatric Medicine Unit is operationally and professionally accountable to the Division and provides medical leadership in their area of specialty across the continuum of care. Services are provided across the following care settings:

- Inpatient services include - Acute Aged Care, Geriatrician in Emergency Department and Rapid Assessment Medical Unit, Orthogeriatrics, Cardiogeriatrics, Geriatric Evaluation and Management, Dementia Management Unit, Rehabilitation, Transitional Care, SNAP.
Specialist Clinics include - Continence, Falls, CDAMS, Wound Management, and Movement Disorders.

The Unit has a strong focus on undergraduate and postgraduate education and promotes a close liaison with regional partners. The service fosters person centred care at all levels, is focused on innovation in service delivery, and is committed to providing safe, high quality care to our community.
