At My Companionship, we’re an Australian-based, locally managed home care provider. We offer personalised, face-to-face support that empowers older Australians to live safely and independently in their own homes for longer via Home Care Packages.

As we continue an exciting growth trajectory, we’re looking for a passionate Care Manager to join our team. This full-time **work-from-home** role offers the best of both worlds, the flexibility to work remotely while also engaging directly with clients through face-to-face visits as part of our commitment to personalised care.

**Key Responsibilities**:

- Develop and update client care plans in collaboration with clients and their families.
- Ensure compliance with all relevant regulations and company policies.
- Serve as the main point of contact for clients and their families, and support workers addressing any concerns or issues promptly.
- Conduct regular reviews of client care and make necessary adjustments to care plans.
- Collaborate with healthcare professionals to ensure the best outcomes for clients.
- Oversee the management of client budgets and ensure the efficient use of resources.
- Provide support to care staff to ensure high standards of care.

**Qualifications**:

- Relevant qualifications in Nursing, Social Work, or a related field.
- Proven experience in a care management role, preferably in a home care setting.
- Strong leadership and team management skills.
- Excellent communication and interpersonal abilities.
- Knowledge of care regulations and best practices.
- Ability to handle sensitive situations with empathy and professionalism.

Pay: $80,000.00 - $88,000.00 per year
