This role works with families, referrers, and services to effectively co-ordinate services for children with developmental concerns to ensure the right service provision, at the right time and in the right place.
**The role includes**:

- Triaging of referrals for the Child Development Service (including specialist child development service, paediatric outpatient clinics and sub-acute care)
- Comprehensive knowledge of referral and eligibility guidelines, scheduling processes/guidelines.
- Cross department review of referrals to ensure 'no wrong door' for children that are referred to General Paediatrics.
- Initial referral response for families who require additional support to engage with services whilst waiting for care.
- Engagement with referrers in West Moreton catchment to support timely and appropriate referrals.
- Engagement with services across the West Moreton catchment that work with children with developmental concerns to ensure alternative pathways of support where appropriate.

About you
- High level clinical skills with experience and knowledge of children with developmental disorders and their families across the developmental continuum 0-18 yrs.
- Knowledge of referral and scheduling processes/guidelines
- Ability to operate effectively within an integrated interdisciplinary team model inclusive of medical and allied health personnel, across hospital and community-based settings and contribute to effective partnership development across the government and non-government sector.
- Knowledge of frameworks and guidelines and policies that support an inclusive culturally responsive environment for clients, referrers, and colleagues.
- Ability to integrate service changes into practice and engage in quality and service improvement activities to ensure high quality service provision for children and families.
