**ABOUT THE ROLE**:
Are you looking for a unique opportunity to develop your skills in a community environment? Do you want to be part of a dynamic team that is seeking to improve people’s lives? SwanCare is looking for a suitably qualified person to work in our Community Living team using their skills to provide high level customer service, provide information on and participate in providing support services to a variety of independent living residents as well as working within the home care team providing assistance to the manager and clients.

Monday to Friday role. The Residential Clinical Care Nurse (Resident Support Officer - Clinical and Care) is an amazing opportunity for someone who loves variety in their day. Your responsibilities may include but are not limited to:

- Provide day-to-day support requirements for village residents including, emergency calls (falls, serious illness, deaths, crisis support and lock-outs) and equipment requirements (support to purchase or rent).
- Complete necessary documentation as required, ensuring relevant progress notes are recorded in a manner which is compatible with requirements such as ensuring they are factual, concise, accurate and objective.
- Liaise with relevant GP’s and allied health services on resident support where appropriate and support residents in accessing appropriate medical and support services.
- Undertake assessment and reassessment of home care clients as required.
- Assist Manager SwanCare At Home with service enquiries, onboarding of new clients and other duties related to home care.

**Skills and Experiences**:
**KEY ROLE REQUIREMENTS**:

- AHPRA registration
- Case coordination experience in a home care setting, particularly in complex care
- Understanding of and ability to manage daily rosters when required
- Valid ‘C’ class driver’s licence
- Working rights within Australia
- National Police Clearance less than 6 months old and NDIS Worker Screening Check
- Current Influenza Statement
- COVID-19 Vaccination
- Well-developed IT literacy skills that include the MS suite of products; specifically MS Word, Excel, Access and Outlook
- Ability to maintain non-disclosure of confidential and sensitive information that may be critical to operational and financial success
