**Outreach Mental Health Support Worker**

**About the Role**:
Conscious Care Support is seeking dedicated **Outreach Mental Health Support Workers** to provide psychosocial, emotional, and practical support to individuals with chronic psychosocial disabilities. In this role, you’ll:
✔ Work independently in clients’ homes and the community
✔ Support individuals in identifying and achieving their goals
✔ Collaborate with care teams for high-quality, client-centred support.

This is an **outreach role**, meaning you’ll work autonomously while still being part of a supportive, tight-knit team.

**What You Need**:
✔ **Cert IV in Mental Health (minimum)**
✔ Strong case note writing skills (concise & articulate)
✔ In-depth understanding of mental health and psychosocial disability
✔ A genuine passion for empowering others
✔ A driver’s license & comprehensively insured vehicle
✔ A mobile phone for communication with clients and stakeholders

**Additional Requirements**:
✔ **NDIS Workers Screening Check**
✔ **Police Check (from within the last 12 months)**
✔ **Working with Children Check (WWCC)**
