**About the role**

The difference you can make includes supporting our customers who are all very individual and have varying needs. You will need to be prepared to support customers with personal care, going to medical appointments, to work and various community activities such as sports, walking, shopping and cooking. An outgoing, vibrant personality will see you achieve these outcomes. Working hours in community are Monday to Friday with hours between 9am and 2pm.

A typical day in the role may look like
- Delivering a wide range of domestic supports within a customer’s residence and community;
- Supporting person-centred plans that identify a customer’s life goals;
- Supporting customers to engage in community-based activities;
- Ensuring positive behaviour support plans are followed; and
- Administration duties and mandatory daily reports.
- Well-developed interpersonal skills with the ability to build effective working relationships;
- Strong communication skills;
- Ability to follow person-centred plans and assist people to achieve their goals and outcomes;
- Experience in manual handling;
- Willingness to assist with personal care and domestic duties;
- Cert III in Disability or Individualised Support desirable;
- Proven administration and computer literacy skills;
- Valid WA drivers licence and a willingness to use your private vehicle (aligned with Activ’s motor vehicle allowance rebate).
- Police clearance and/or NDIS worker screening check.
- NDIS Worker Orientation Module

**Activ staff working in roles that require regular attendance to our residential disability accommodation homes must be fully vaccinated against COVID-19 and receive a booster dose within one month of becoming eligible.**
