We offer flexible and hybrid work options and genuinely care about providing our staff work/life balance. **ABOUT THE ROLE**: We have two exciting positions for Age Well Planners, you can be part of the development of the New Moreland Navigation Service. The Age Well Planners will be responsible for supporting Moreland residents to be able to access the services they need, by increasing their knowledge of My Aged Care, developing an Age Well Guide, providing regular "Ask us Anything" sessions In the community as well as one on one support via the phone or In person and at our local events and festivals. As well as regular evaluation and reporting of progress. **SKILLS & EXPERIENCE REQUIRED TO PERFORM THIS ROLE**: - A degree or diploma in an allied health or relevant profession (e.g. health promotion, community development or social sciences) or equivalent mix of lesser formal qualifications and substantial experience working with diverse older people or people requiring additional support - Experience working in the health or community services sector - Sound knowledge of aged care service system, social determinates of health in older people, policy directions with the aged care sector and ageing well principles - An applicant with lived experience of the aged care system or a self-identified older person - Familiarity with the Moreland community and service organisations - Ability to speak a language other than English that is relevant to the older Moreland community (such as Italian, Greek or Arabic) **WHAT THIS ROLE CAN OFFER**: - The opportunity to work alongside a professional and supportive team - Salary sacrifice options - Extensive opportunities for training and career development - Wellness programs For further information please contact **Tanya Taylor on 03 9240 2302** or **Shiona Novkovic-Heaven** on** 03 9240 2338** **How to apply**:
