Nextt is seeking a compassionate and experienced **Support Worker** to provide in-home and community-based support to an individual living with dementia. This is a great opportunity to join a values-driven organisation focused on empowering people to live independently and with dignity.

**Key Responsibilities**:

- Provide person-centred care including personal care and medication assistance.
- Support the client during medical appointments and community access.
- Drive a modified vehicle (experience preferred).
- Promote independence and choice in all aspects of care.
- Work autonomously while maintaining communication with the broader support team.

**Preferred Experience**:

- Experience supporting clients with dementia and aged care needs.
- Competency in personal care and medication support.
- Experience in community engagement and transport.
- Familiarity with using or driving modified vehicles.

**Mandatory Requirements**:

- Current Australian Driver’s Licence
- NDIS Worker Screening Check (or willingness to obtain)
- Working with Children Check (if required for the role)
- Current First Aid and CPR certification
- Right to work in Australia

**Roster Line 3 - 57 Hours Per Fortnight** **Week 1**:

- Monday: 4:00pm - 10:00pm
- Wednesday: 4:00pm - 10:00pm
- Thursday: 5:00pm - 10:00pm
- Friday: 4:00pm - 10:00pm

**Week 2**:

- Monday: 11:00am - 5:00pm
- Tuesday: 5:00pm - 10:00pm
- Wednesday: 5:00pm - 10:00pm
- Friday: 5:00pm - 10:00pm
- Saturday: 4:00pm - 10:00pm
- Sunday: 3:00pm - 10:00pm

**Pay Rates (as per applicable award)**:

- **Day Shift (Base Rate)**: $33.03/hr
- **Afternoon Shift (Ends between 8pm and midnight, Mon-Fri)**: $37.16/hr
- **Active Night Shift (Finishes after midnight or starts before 6am, Mon-Fri)**: $37.98/hr
- **Saturday**: $49.55/hr
- **Sunday**: $66.06/hr
- **Public Holiday**: $82.58/hr
