
Ndis Support Coordinator

**Key Responsibilities**:
- **Assessment and Planning**: Conduct comprehensive assessments of clients’ needs, strengths, and preferences. Develop and implement individualised support plans in collaboration with clients and stakeholders.
- **Resource Coordination**: Identify and connect clients with appropriate community resources, services, and support systems, including healthcare, housing, employment, and social services.
- **Advocacy**: Act as an advocate for clients, ensuring their rights are upheld and their voices are heard. Provide guidance and support in navigating complex systems.
- **Monitoring and Review**: Regularly monitor and evaluate the effectiveness of support plans, making adjustments as needed to ensure clients’ goals are met.
- **Documentation**: Maintain accurate and confidential records of client interactions, assessments, and progress notes. Prepare reports as required by management or funding agencies.
- **Collaboration**: Work closely with multidisciplinary teams, including social workers, healthcare providers, and community organizations, to ensure coordinated and holistic support for clients.
- **Education and Training**: Provide information and education to clients and their families about available resources, services, and strategies for achieving their goals.
**Job Types**: Part-time, Casual
Pay: $32.19 - $40.00 per hour
