**Position Overview**:
**Key Responsibilities**:

- Assist NDIS participants in understanding and implementing their NDIS plans.
- Coordinate and manage a range of supports for clients, including informal, mainstream, and funded supports.
- Facilitate client choice and control by empowering them to make informed decisions about their supports.
- Develop and maintain strong relationships with clients, families, and other stakeholders.
- Monitor and review the effectiveness of supports and services in achieving client goals.
- Advocate for clients' rights and ensure they receive high-quality services.
- Provide guidance and support in navigating the NDIS system, including plan reviews and changes.
- Maintain accurate and up-to-date records and documentation.

**Qualifications and Experience**:

- A relevant qualification in Community Services, Social Work, or a related field.
- Previous experience as an NDIS Support Coordinator or in a similar role is preferred.
- Strong understanding of the NDIS, including participant plans, goals, and support coordination.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Commitment to person-centered practice and a non-judgmental approach.
- Valid driver's license and access to a reliable vehicle.

Pay: $38.00 - $42.00 per hour

Expected hours: 22.5 per week
