**The Role**
We are seeking an experienced and dedicated **Care Coordinator** to join our Gregory Hills Head Office team on a full-time basis (Monday to Friday).
In this role, you will:

- Coordinate and oversee care plans, risk assessments, and service agreements.
- Supervise, mentor and support a team of Support Workers.
- Conduct client onboarding, home visits, and quarterly check-ins.
- Collaborate with clients, families, and stakeholders to ensure person-centered outcomes.
- Manage client documentation, incidents, complaints, and continuous improvement processes.

**What We’re Looking For**
✔ Minimum 2 years’ experience in **care coordination** or **case management**
✔ Strong knowledge of NDIS, disabilities, **complex care and mental health.**
✔ Proven leadership and staff supervision skills.
✔ Excellent communication, organisation, and time management skills.
✔ Current Working with Children Check, NDIS Worker Screening, Police Check, Driver’s Licence, and access to own vehicle.
✔ Relevant tertiary qualification in Disability, Allied Health, Social Science or similar (highly regarded).
