**About Us**

Palliative Care South East provides palliative care in the south east metropolitan region of Melbourne. We support people with a life-limiting illness to live well and, in doing so, provide support at home to improve the quality of life of individuals, carers and families. Our specialist palliative care services are provided to people of all ages, free of charge, in the Local Government areas of Cardinia, Casey, Greater Dandenong and part of Kingston.

**The Role**

Working within an interdisciplinary team, the Team Leader 3B will provide day to day leadership to the operational team by providing mentorship with a strong focus on staff development. The Team Leader will lead a group of clinicians in accordance with the two-team model and provide day to day professional clinical support and education to colleagues. The Team Leader role has responsibility for triaging clients onto the service and managing crisis calls from clients and carers.
- Some of your responsibilities will include:_
- Support and provide professional and clinical oversight to the team to develop complex care plans
- Assist in day-to-day planning or resources to ensure continuity of care with clients
- Manage own clinical case load within team model on daily basis
- Provide comprehensive guidance, accurate assessment and problem-solving opportunities with colleagues
- Provide clinical advice and support to the team in delivering quality care for all clients including where complexity exists
- Partner with interdisciplinary team clinical professional leads to appraise performance to ensure clinical care is provided from an interdisciplinary base.

**Required Skills and Experience**
- Registered Nurse (Division 1)
- Postgraduate qualification in palliative care or working towards
- Demonstrated 3 years palliative care experience
- Current registration with Australian Health Practitioner Regulation Agency
- Demonstrated leadership experience including change, coaching and development, motivating self and others and managing performance
- Excellent Communication and Interpersonal skills both written and verbal
