The Out of Hospital Care Medical Clinical Director
- Hospital in the Home (HiTH) / InTouch is a critical role that provides medical leadership, oversight and clinical care to patients receiving Out of Hospital Care within the WSLHD catchment area. This role encompasses significant responsibilities including strategic, operational and clinical governance to ensure that services operate efficiently and maintain the highest standard of care. The position will contribute to the development of innovative models of care to evolve Integrated & Community Health care services to meet future health service delivery challenges.

The Medical Clinical Director works collaboratively across departments to enhance care delivery and optimise clinical outcomes and support early discharge and hospital avoidance strategies.

This role ensures the service aligns with the direction of the WSLHD strategic plan and the Ministry of Health strategic directions, policies and key performance indicators.

The Western Sydney Local Health District (WSLHD) Hospital in the Home (HiTH) is a well-established and mature service that delivers safe, high-quality hospital substitution care to patients in the comfort of their own homes. Operating as a fully integrated component of the acute care system, WSLHD HiTH plays a critical role in supporting patient flow and enhancing hospital capacity.

In 2024, the service treated over 3,500 patients who would otherwise require admission to a traditional inpatient setting. These patients receive comprehensive multidisciplinary care for a wide range of acute conditions, including infections requiring intravenous antibiotics (such as cellulitis, pneumonia, and urinary tract infections), post-operative care, wound management, and other complex clinical needs that can be safely delivered in a home environment.

Since April 2022 the WSLHD inTouch Urgent Care Service has provided episodic urgent care services to persons in their place of residence. This has been achieved by finding a real-time alternative care pathway such as a rapid response referral to Hospital in the Home (HITH), outpatient clinics, mobile imaging, and providing medical navigation to care through collaborative assessment and consultation with other specialties. A total of 4,007 ED avoidances have been achieved since implementation.

With a strong focus on person-centred care, early discharge, and admission avoidance, WSLHD HiTH and inTouch continues to demonstrate excellence in delivering hospital-level care beyond the walls of the hospital.

**Selection Criteria**
- Possess or be eligible for registration with the Medical Board of Australia and Fellowship of the Royal Australasian College of Physicians or Royal Australasian College of General Practitioners or equivalent as provided for in the Staff Specialists (State) Award/ Health Insurance Act 1997. Must have Australian Citizenship or Permanent Australian Residency.
- Sound contemporary clinical knowledge in Emergency Medicine Geriatric Medicine, Infectious Diseases, Respiratory Medicine and, general practice / general medicine demonstrated through successful recent clinical practice in an environment appropriate to the requirements of the position with a focus on hospital care avoidance and substitution
- Advanced ability to work as part of an executive team to deliver strategic goals whilst leading and working effectively as part of a multi-disciplinary clinician team in the planning, delivery and coordination of patient care
- Demonstrated personal qualities of clinical leadership and service development, including advanced organisational and time management skills and ability to manage competing priorities in a complex environment.
- Demonstrated experience leading change and implementing new initiatives whilst supporting and facilitating a workplace culture that supports positive and respectful behaviours, high performance, and accountability.
- Demonstrated clinical competency in utilising digital technologies for safe and quality healthcare delivery and the adoption by other clinicians in the service.
- Demonstrated skills and attitudes appropriate for practice as a Senior Medical /Dental Practitioner including:

- Evidence of Continuing Medical Education and self-monitoring/audit practices that are commensurate with the Scope of Practice requirements of the position.
- o High level interpersonal and advanced communication skills, including the ability to convey, and adapt verbal and written information to a clinical and non-clinical audience
- Experience and involvement in quality improvement, patient safety, risk management and translating evidence-based best practice.
- Demonstrated experience and commitment to teachin
