**About the Service**

We are excited to launch a new in-reach rehabilitation model of care for stroke patients at Box Hill Hospital. This innovative service aims to optimise rehabilitation access by providing early assessment and intervention for patients who are awaiting transfer to rehabilitation, requiring rehab but not yet ready for subacute care, or those who can be discharged directly home with support. Working in partnership the award-winning acute stroke multidisciplinary team, the in-reach team will enhance patient flow and support high-acuity care by commencing timely, evidence-based rehabilitation interventions.

**About the role**

The stroke service is an innovative and growing area with a dedicated mutidisciplinary team working closely with patients to provide excellent centred care in support of the patients journey back into the community. The Grade 3 Allied Health Assistant role offers a great opportunity to work closely with the Allied Health and broader multidisciplinary team in delivering this care alongside other AHA's with tasks delegated by the Allied Health Professional team members

**About you**

To be successful in this role you will possess:

- Essential:_
- Completion of the Certificate 4 (Allied Health Assistant) from a registered training organisation including training modules in one or more of Occupational Therapy, Physiotherapy, Speech Pathology, Dietetics, Social Work, Podiatry or undergraduate Allied Health Professional training in one of these professions where commitment to a regular roster can be assured.
- 3 years experience working as a full time Grade 2 AHA or alignment as an Allied Health Professional student
- Well-developed verbal & written communication skills to enable effective liaison with a multidisciplinary team including effective clinical handover & engagment in the AHA delegation process
- Well-developed clinical competence and sound theoretical knowledge base relevant to the workplace
- _Desirable:_
- Experience either as an AHA or Allied Health Professional student in working in a hospital based or healthcare environment
- Experience either as an AHA or Allied Health Professional student in working in the clinical area of neurology
- Healthcare workers are strongly recommended to follow COVID vaccination recommendations provided in the _Australian Immunisation handbook (based on ATAGI advice)_. Seasonal vaccination against influenza is a mandatory requirement of this role and an offer is conditional on this being up to date prior to employment._
