**NFP Home Care Provider and well-known Community Organisation.**

Are you an
**innovative, passionate individual **who has a background in
**Home Care packaging/ Support coordination?**

You will play a pivotal role in supporting home care clients from intake through to management. Be part of a team that is dedicated to making a positive impact in the lives of seniors and individuals with varying care needs.

You will be required to work across the west Sydney area and attend the office 2 days a week based in Blacktown.

**Duties**
- **Assessment and Planning**: Conduct thorough assessments to understand the unique needs of clients and collaborate with them to create personalized care plans.
- **Client Engagement**: Build strong relationships with clients, their families, and support networks to ensure a holistic and compassionate approach to care.
- **Educational Support**: Empower clients with knowledge about available services and options, guiding them through the home care package process.
- **Documentation and Reporting**: Maintain accurate records of client interactions and progress, ensuring compliance with organizational standards.
- **Collaboration**: Work closely with internal teams and external stakeholders to provide seamless care coordination.

**About you**:

- Experience within Home Care Packaging/ Care Coordination
- Strong communication and documentation skills
- Knowledge of aged care guidelines and home care packaging
- Passion for helping others
- Ability to work great in a team
- Ability to work towards KPI's
