Are you an experienced, compassionate, and outcomes-driven Support Coordinator looking for a flexible, part-time role where your work can create meaningful impact in someone’s life? **Sovereign Lives Victoria** is currently seeking a skilled and dedicated Support Coordinator to join our team in supporting an NDIS participants.

**Key Responsibilities**
- Deliver person-centred support coordination in line with NDIS goals and guidelines
- Engage in direct communication with the NDIS
- Assist with developing, monitoring and refining service agreements and schedules
- Help the participant build capacity to support independent living
- Provide consistent progress updates and support outcome reviews
- Identify, reduce, and manage risks within the participant’s support environment and
- Provide general service coordination support and advocacy when needed

**About You**
- Minimum 2 year of experience in NDIS Support Coordination or a similar role
- Demonstrated success in delivering strong results and positive participant outcomes
- Proven ability to manage time, priorities and communications across multiple stakeholders
- Fully trained with a sound understanding of the NDIS framework
- Flexibility in service delivery.
- Strong interpersonal skills, with a compassionate and culturally sensitive approach

**How to Apply**

**Job Type**: Part-time

Pay: $40.00 - $45.00 per hour

Expected hours: No more than 15 per week

**Experience**:

- Support Coordination: 2 years (required)

Licence/Certification:

- NDIS Screening Check (preferred)
- Full Driver Licence (preferred)

Work Authorisation:

- Australia (required)

Work Location: In person
