**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

**About the role**
- Barwon Health in Geelong is seeking a Health Navigator to join the team as part of the Pathways to Good Health Initiative. The Navigators role is to build relationships with the local Child Protection practitioners to collaborate and improve the health outcomes for Children in Out of Home Care (OOHC) who are known the Department of Families, Fairness and Housing (DFFH) and Child Protection (CP).
- To be considered for this opportunity, you will have the following:
- Able to undertake triage and assessment of a child’s health needs.
- Ability to work collaboratively
- A fast, active work pace and capacity to handle large volumes of work
