Care & Connects is an innovative NDIS provider dedicated to delivering the best quality support to each of our participants. To assist with this, we have a dedicated Psychosocial Disability program led by experienced Mental Health professionals, delivering Supported Independent Living (SIL) and Community Participation (CP) supports across the Adelaide region.

**About you**:
We are looking for an experienced Client Intake Coordinator to play a vital role in supporting participants and ensuring high-quality service delivery.

**Qualifications**
- Qualification: Minimum Certificate III in Disability, Mental Health, Community Services (Diploma/Bachelor’s preferred)
- WWC & NDIS Worker Screening
- AU Driver’s License
- Police Check
- 100 Points ID

**Selection Criteria**
- 2+ years in disability services, intake coordination, or a similar role
- Strong communication, stakeholder engagement, and data management skills

**Your tasks may include but not be limited to the following**

Engaging with new participants, conducting intake assessments, and coordinating services
- Managing referrals and onboarding participants for accommodation, community access, and support services
- Building strong relationships with participants, families, and stakeholders
- Ensuring compliance with NDIS regulations and organisational policies
- Promoting Care & Connects through networking events and inter-agency meetings Any administrative & reporting tasks as and when required by Management

**How to Apply?**

Pay: $70,000.00 - $80,000.00 per year

Schedule:

- Monday to Friday
- No weekends

Work Authorisation:

- Australia (preferred)

Work Location: In person
