Support Coordinator

**About the Role**
We are seeking two experienced and dedicated **Level 3 Support Coordinators** to join our growing team in **Clyde, Melbourne**. In this role, you will work with participants of the **National Disability Insurance Scheme (NDIS)** to build capacity, coordinate supports, and help them achieve their individual goals.
As a Support Coordinator, you will assist participants in navigating the NDIS system, liaising with service providers, and ensuring that support plans are implemented effectively and in alignment with participant needs.
**Key Responsibilities**
- Coordinate and manage complex NDIS participant supports (Level 3 Coordination).
- Build participants' capacity to understand, implement, and manage their NDIS plans.
- Connect participants with appropriate community, mainstream, and funded supports.
- Monitor and review service delivery to ensure quality and participant satisfaction.
- Maintain accurate case notes and reports in compliance with NDIS standards.
- Collaborate with families, support workers, and allied health professionals.
**Essential Requirements**
Relevant qualification in **Community Services, Social Work, Psychology, or Disability Studies** (minimum Certificate IV or higher).
Demonstrated experience as a **Support Coordinator** or in a similar case management role.
Comprehensive understanding of the **NDIS framework, policies, and processes**.
Completed **NDIS training and Worker Orientation Module**.
**Current Australian Driver's Licence** and access to a reliable vehicle.
**Full working rights in Australia** (citizen, permanent resident, or valid visa).
Valid **NDIS Worker Screening Check** and **Working with Children Check (WWCC)** (or willingness to obtain).
Minimum of 3+ years of experience as a Support Coordinator or in a similar case management role.
