**About us**
DHC works in partnership with individuals, their families, mainstream providers, and the disability sector to ensure individual services are innovative, flexible, and person-centred. We service the Perth metro area (and other WA regions) with services ranging from in-home support to community access. As a Service Coordinator, you will work in partnership with our clients to identify their goals and aspirations; and to support them achieve their goals and desired outcomes by understanding their individual needs as funded through their NDIS Plan. This casual role reports to the Manager and is responsible for delivering Service Coordination services to our expanding client base.
**Essential criteria**
- Relevant qualifications/certifications and/or significant experience within the NDIS framework and case management role
- Current Western Australian driver's license and vehicle with comprehensive car insurance
- NDIS Worker Screen Check
- Working with Children Check
- Completion of the NDIS Worker Orientation Module
- First Aid Certificate
- Fully Covid-19 vaccinated
**Skills and Experience**
- Strong people skills and the ability to communicate effectively with a range of stakeholders.
- Excellent written and verbal communication skills, including proficiency with client management systems and competence with the Microsoft Office suite.
- Effective problem-solving, communication, and organisational skills.
- Excellent time management skills, including the ability to effectively manage multiple projects and effectively set priorities.
- An in-depth understanding of a person-centred approach to developing individualised support strategies and outcomes in partnership with clients, family members, stakeholders, and support staff.
- Ability to work directly with the client, family/carers, and relevant stakeholders to assess the quality and value of the support services provided and ensure they meet the client’s individual needs.
- A demonstrated understanding of the NDIS practice standards, NDIS plans and pricing, budget development, and service agreements.
- An understanding of the NDIS Quality and Safeguards Framework.
- Reviewing, recording, and resolving client complaints and compliments fostering a culture of continuous improvement.
- Recognise, respond, and adapt to changing demands in the work environment that impact the health and safety requirements of the role and effectively manage risks.
- Demonstrated experience working with people with complex needs and navigating high-level risks to overcome complex barriers.
- Experience managing and supporting a team of Disability Support Workers to promote a cohesive team and support a high standard of service delivery.
