**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

**About the role**
- As the Community Nurse (CN) liaison you will form part of the Complex Planning and Pathways team and will be responsible for the identification and facilitation of patients who will be discharged from bed-based services to the Community Nursing program with appropriate supports in place. As the CN liaison officer will also provide monitoring of current Community Nursing patients in bed-based services and ensure adequate and timely discharge planning in preparation for transition to the community setting.
