About the facility

Our major sites include University Hospital Geelong in Geelong's CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon Southwest Region, including Torquay and Colac.

About the program

Our program supports clients and carers in their home environment to achieve an optimum level of health independence through the development and delivery of a high quality, cost effective, locally relevant Community Nursing service. We provide Palliative Care (CPC), Post-Acute Care (PAC), Commonwealth Home Supports (CHSP), Hospital in the Home (HITH) & Home and Community Care (HACC) nursing services.

About the role

Are you interested in working in Community Nursing (District Nursing)? Are you available to work weekdays, weekends and evenings? If so, then this opportunity with the South (Belmont) Community Nursing Team at 32 hours per fortnight may be the role for you. There is opportunity to work additional hours if you choose. Applicants from outside the Community Nursing program will be supported with a thorough and supportive orientation process.

In this role you will be responsible for the provision of patient focused holistic nursing care to those assigned to you in order to achieve desired clinical outcomes. You will have highly developed interpersonal skills as well as sound communication skills both verbal and written.

The successful applicant will need to have current/recent acute or community health experience. Experience & knowledge in the areas of wound care, palliative care, diabetes and continence management and community health care principles are desirable.
