**About the Role**

Care and Love Pty Ltd is seeking an experienced and compassionate **NDIS Independent Support Coordinator** to join our team. This role is ideal for professionals who are passionate about empowering people with disabilities to live independently and reach their goals through the National Disability Insurance Scheme (NDIS).

You will be the key liaison for clients, families, and service providers, ensuring that supports are effectively coordinated, aligned with participants’ needs, and responsive to their evolving goals.

**Key Responsibilities**
- Develop and manage individualised NDIS support plans in partnership with clients and their families/carers
- Coordinate and monitor service delivery across multiple providers
- Build and maintain strong relationships with clients, providers, and stakeholders
- Assist clients in understanding and navigating the NDIS system
- Regularly review plans and progress, making updates as needed
- Maintain accurate and compliant records in line with NDIS and organisational requirements

**What We're Looking For**
- Tertiary qualification in community services, social work, disability, or a related field (preferred)
- Minimum 2-3 years experience in NDIS Support Coordination or similar role
- In-depth understanding of the NDIS framework, guidelines, and best practices
- Strong interpersonal and communication skills
- Highly organised with a focus on client outcomes and problem-solving
- Proficiency with client management systems and digital tools
- Self-motivated, with the ability to work independently and in a team
- Valid driver’s licence and willingness to travel locally as needed
- Full working rights in Australia
