To be considered for this exciting opportunity, you will need to possess the following qualifications and experience:
**Minimum Requirements**
- Certificate III or above in Individual Support (Disability or Aged Care)
- National Police Check Clearance
- Working with Children Check (Known as Blue Card)
- NDIS Worker Clearance Certificate or Yellow Card
- Experience in working with people with challenging behaviours.
- Experience in mental health
- Experience in safe handling of food
- Experience in safe use of Medication
- Certificate of NDIS Orientation Module
- COVID-19 Vaccine Certificate (Fully vaccinated)
- 2 x References
- Current QLD Driver’s License
- Comprehensive Car Insurance
**Key Responsibilities**
- Follow individual support plans, behaviour support plans, and health care plans.
- Participate in community activities within a Supported Independent Living environment.
- Maintain and monitor a client’s support plan whilst modelling a person-centred approach.
- Identify and support a range of learning opportunities for our clients.
- Observe and document an individual’s progress and any changes through detailed note-keeping.
**Provide and facilitate good working relationships with all team members**:
- Monitor the safety and well-being of clients and report any concerns to relevant supervisor/manager.
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes and policy review.
- Ensure relevant information about client or client behaviour is communicated directly if time sensitive.
- In conjunction with leadership team, when requested assist in developing, implementing and evaluating training and behaviour programs to attain client identified individual goals.
- Liaise extensively with DSWs to ensure the overall smooth and consistent running of the service.
- Report regularly to supervisor/manager.
- Contribute to reviews, assessments and consultation when requested by HelpAlong Pty Ltd.
- Ensure a safe working environment for yourself, your colleagues and members of the public.
- Take reasonable care that own acts or omissions do not adversely affect the health and safety of clients or other persons.
- Report any quality and safety incident, or hazards at work, to Service Manager.
- Follow responsibilities and duties as detailed in relevant policies and processes.
- Maintain qualifications, ongoing training and further education, as required to fulfil role and duties.
**Please note**:Due to the support needs of our clients, you must be available and willing to work a varied pattern of shifts across a regular 24/7 roster (including short shifts, night shifts, weekends, and public holidays) to be considered for these roles.
**Job Types**: Full-time, Part-time, Permanent, Casual
Part-time hours: 20-40 per week
**Salary**: $30.00 - $84.70 per hour
