All Aged Care is a quality provider of in-home care services located in Brisbane CBD close to public transport with company cars available for travel to clients homes when required. We are a multi-cultural organisation who pride ourselves on delivering services which exceed client's expectations which enable clients to remain living independently in their own homes.

We are seeking a self-motivated **Home Care Intake Officer **who can work autonomously as well as part of a multi-disciplinary team and enjoys building good relationships with new clients and their families. This role is responsible for inbound referral management, meeting and signing up potential new clients and facilitating handover of the clients to the allocated coordinator who takes the client relationship forward.

To be successful in this role you will have:

- Experience and a solid understanding of home care packages and CHSP programs
- A motivated people person who enjoys meeting new and different people daily, with excellent communication and interpersonal skills and a "can do attitude
- Good networking and presentation skills with an ability to promote our services to hospitals, GP's and community groups
- Good computer skills including care databases, Outlook and MS suite of products
- An ability to speak another language is highly regarded

You will need to be a permanent Australian resident, have a National Police Check (or willingness to obtain), open Australian Drivers Licence. You are required to be fully vaccinated - COVID19 and Influenza.

**Salary**: $60,000.00 - $70,000.00 per year
