**About the role**:
We are seeking a Community Case Manager to join our team and play a vital role in supporting elderly people in the community to live independently and with dignity in their own homes.
As a Community Case Manager, your primary responsibility will be to manage a portfolio of community-based elderly customers with Home Care Packages. You will assess their needs, develop personalised care plans, and manage budgets to ensure each customers' unique requirements are met. Your role will also involve signing up new customers and proactively managing existing ones, whilst maintaining a person-centred approach. Collaboration and coordination with Nurses and Allied Health professionals will be crucial to delivering comprehensive care.
In this full-time, 12-month contract position, you will have the flexibility to manage your own schedule each week, whilst working autonomously from home and within the community (approx. 50/50 split). You will work with customers in the southern suburbs of Tasmania, including Margate, Derwent Valley, Rokeby, Bridgewater, and more.
**About You**:
- Previous experience in community-based case management is preferred.
- Experience with Home Care Packages is desirable.
- Current driver’s licence and a reliable vehicle with insurance.
- Strong communication skills, both verbal and written.
- Demonstrated empathy and compassion.
- Excellent problem-solving abilities and critical thinking skills.
- Good organisational and time management skills.
- Cultural competence and the ability to work with diverse populations.
- Advocacy skills to ensure clients' needs are met.
- Collaboration and teamwork skills to work effectively with interdisciplinary teams.
