About the Role
- Leading and managing specialised community-based services (including services of a complex nature). Including the provision of operational advice to multi-disciplinary teams. This includes the provision of clinical practice supervision relevant to professional scope and background.
- Facilitate evidence-based practice in the area of interprofessional team members in the context of the Community Services Team (CST) to ensure efficiency and effectiveness of service provision and discharge planning, through implementation and facilitation of quality initiatives, literature reviews, attendance at professional development activities and involvement in research and education.
