Job Summary:
Provide person-centered, flexible, and responsive support to individuals with complex needs, promoting their independence, well-being, and social inclusion. Work collaboratively with clients, families, healthcare professionals, and other stakeholders to develop and implement tailored support plans.

Key Responsibilities:
1. Client Support: Provide emotional, practical, and behavioral support to clients, respecting their autonomy, dignity, and individuality.
2. Support Planning: Collaborate with clients, families, and healthcare professionals to develop, implement, and review personalized support plans.
3. Risk Management: Identify, assess, and mitigate risks associated with clients' complex needs, ensuring their safety and well-being.
4. Communication: Develop and maintain effective communication strategies, adapting to clients' individual needs and preferences.
5. Collaboration: Work with interdisciplinary teams, including healthcare professionals, social workers, and family members, to ensure comprehensive support.
6. Crisis Intervention: Respond to crisis situations, providing calm and effective support to clients and their families.
7. Documentation: Maintain accurate, up-to-date records of client interactions, support plans, and progress.
8. Professional Development: Participate in ongoing training, workshops, and professional development opportunities to enhance skills and knowledge.

**Requirements**:
1. Qualifications: Certificate IV in Disability, Mental Health, or a related field (desirable).
2. Experience: Minimum 1 years of experience working with individuals with complex needs, disabilities, or mental health conditions.
3. Skills:

- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work flexibly, adapt to changing situations, and prioritize tasks effectively.
- Strong understanding of person-centered support, trauma-informed care, and recovery-oriented practices.
- Ability to maintain confidentiality, professionalism, and boundaries.
4. Clearances: Valid police clearance, working with vulnerable peoples card, and any other relevant clearances.

Working Conditions:
1. Work Environment: Community-based settings, including clients' homes, residential facilities, and community centers.
2. Work Hours: Flexible hours, including evenings, weekends, and public holidays (as required).
3. Physical Demands: Moderate physical demands, including manual handling, lifting, and transferring clients (if required).

**Job Type**: Casual

Pay: $23.23 - $53.07 per hour

Expected hours: 5 - 10 per week
