**City Hub Coordinator**
- ** Full Time and Part Times hours available**:

- ** Permanent position**:

- ** Great opportunity for Senior Nurse to join our Home First Team**:

- ** Centrally located at the Royal Melbourne Hospital - Parkville**
- The service will incorporate two shifts:

- Afternoon shift 4pm - 10pm (6 hours )
- Night shift 9.30 pm - 8 am

**Your new role**:

- Provide advice and support for patients Work in collaboration with medical staff supporting the services
- Support follow up and handover of patients issues and concerns by the appropriate team
- Plan transfer of patients, where required, to the appropriate emergency department
- Support service improvement with data collection, reporting and analysis

**About you**:

- You have extensive acute hospital experience with a collaborative working style
- You are looking to expand and extend your nursing career
- You have excellent communication skills and a passion for continued professional development
- You are an AHPRA registered Nurse with ward experience and are willing to work on a rotating roster
