**About the Role**
- **Full-time permanent (38 hours per week) based in Parramatta LGA**:

- **SCHADS Award level 401, $85,146 per year plus salary packaging $18,550 plus 11.5% Super**:

- **Monday to Friday 7:30am to 4:30pm or 8am to 4pm**:

- **Can independently manage the safe house during the day and all the day-to-day tasks**

The Family Specialist, Case Manager position focuses on providing prenatal support to young expectant mothers within crisis-supported accommodation. The Family Specialist Case Manager will provide a range of targeted education and support programs to mothers in response to identified needs such as Preparation for Labour and Birth, Newborn Baby Care and Support for new mothers.

The program works with young pregnant mums and young mums with one child. The specialist Case Manager will provide a wide range of individual, parent-infant therapies to women and their families.
- Specialist Homelessness Service SHS Youth provides crisis supported accommodation with case management, for people aged 16-25 years old, who are homeless or at risk of homelessness. Our services provide 24/7 onsite support and assistance with education, employment, referral into counselling services, health services, living skills and parenting programs._

**Duties and Responsibilities include but are not limited to**
- Provide skilled safe, holistic, culturally competent, women-centered care to expectant mothers in collaboration with the multidisciplinary team.
- Create and facilitate a variety of groups and programs (creation of flyers, calendars, manuals and other printed and digital material) regarding available groups
- to target expectant mothers within crisis accommodation, ensuring they meet all compliance indicators and regulations
- Ensure the continuing improvement of the group and individual services delivered by participating in quality improvement processes to promote evidence-based, best practice standards.
- Work collaboratively to maintain effective, sustainable relationships with young mothers and their families/significant others to promptly respond to their mental health and prenatal needs

**What you need to succeed**:
**Qualifications**:

- **_Bachelor’s degree _**qualifications in Psychology, Social Work, or relevant discipline

**_Even Better:_**
- Full registration with AHPRA or eligibility for registration with AASW

**Experience**:

- Three (3) years or more experience in case management or relevant work experience in young people or child protection.
- Experience working with young mothers at risk of homelessness or early childhood
- Strong organisation skills and attention to detail to prepare reports, manage caseload and follow-up appointments
- Ability to work with mothers with complex issues using strengths-based approach.
- Demonstrated experience in liaising with other professionals to develop partnerships and referral pathways.
- Ability to engage with new parents and their significant other/
- Previous experience in working in Perinatal care and with young mothers at risk of homelessness.
- Ability to provide effective assessment and referral for a variety of issues and facilitate young people's access to services that will meet their needs.
- Demonstrated ability to work within a recovery-oriented model, utilising a person/family-centred approach.
- Demonstrated ability and experience in group facilitation.
- Demonstrated experience in liaising with other professionals to develop partnerships and referral pathways.
- Excellent verbal and written communication skills, including the ability to keep accurate documents

**Mandatory requirements**:

- Current driver's licence
- First Aid Certificate
- NSW Working with Children Check (Service NSW)
