About the role
Permanent part time opportunity to provide clinical and non-clinical assistance and support to the Social Worker (Residential Aged Care Pathway) and Director of Social Work, as delegated and supervised to improve the effectiveness and efficiency of patient care at Caboolture Hospital.
About you
- Completion and / or submission of forms and other paperwork relevant to residential aged care placements.
- Liaison with external providers including residential care facilities, community organisations, government and non
- government services.
- Work cooperatively and effectively within the Social Work team environment, interdisciplinary health care teams and with patients/ carers and with community partners to contribute to optimal service provision.
- Actively contribute to the Social Work team through departmental and team meetings, projects and activities.
- Contribute to quality improvement activities with increasing ownership within scope of practice under guidance of Social Worker.
