**Care Coordinator (Social Worker) - Full-Time**

Location: Para Hills (7 days/fortnight) & Nuriootpa (3 days/fortnight)

At Pro-Active Health, we are more than a healthcare provider — we’re a hub for community connection, collaboration, and care. With a strong focus on supporting veterans and the broader community, our multidisciplinary team is committed to improving physical, mental, and emotional wellbeing through personalised and holistic care.

We’re now seeking a full-time Social Worker to take on a dual role as a Care Coordinator and Community Connector, working across Para Hills and Nuriootpa. This role is ideal for someone who loves getting out into the community, leading local events, and making sure no one falls through the cracks.

Both clinics are brand new, purpose-built spaces designed to support best-practice client care and team collaboration.

**About the Role**

You’ll support clients — especially Veterans — by coordinating care plans, linking them to internal and external supports, and helping them navigate health and wellbeing services. Beyond the clinic, you’ll also take the lead on local engagement: from facilitating weekly veteran coffee catch-ups and walking groups, to representing Pro-Active Health at community events, local networks, and stakeholder meetings.

**Your Impact**
- Lead veteran-focused community activities like walking groups, social coffee catch-ups, and wellness check-ins.
- Host and coordinate local engagement initiatives, helping veterans and older adults stay connected, supported, and active.
- Conduct comprehensive assessments and develop personalised care plans in collaboration with clients, families, and health professionals.
- Act as a key liaison between clients, GPs, local organisations, and support networks.
- Build strong relationships with RSL clubs, GP clinics, councils, service providers, and veteran support groups.
- Represent Pro-Active Health at community meetings, networking events, expos, and collaborative forums.
- Maintain accurate documentation and contribute actively to multidisciplinary care planning.

**What You Bring**
- A Bachelor or Master’s Degree in Social Work
- AASW membership eligibility
- Experience or passion for working with veterans, older adults, or people with chronic/complex needs
- Strong interpersonal and networking skills — you love engaging with people and building trust
- Confidence to lead small groups, speak at events, and represent a service publicly
- Ability to work independently across multiple sites while staying connected with the wider team

Pay: $65,000.00 - $80,000.00 per year
