As a Disability Client Care Coordinator for Aussie Life Care, you will play a critical role in helping individuals with disabilities live fulfilling and independent lives. Your main responsibilities will include:

- Assessing clients' needs and developing individualized care plans
- Coordinating with healthcare providers, social services, and other support agencies to ensure clients receive necessary services
- Advocating for clients' rights and needs and working to address any issues that may arise
- Monitoring and tracking clients' progress, including conducting regular follow-up assessments
- Providing support and guidance to clients and their families, including information and referrals to community resources
- Working in partnership with a diverse group of professionals to deliver seamless and well-rounded care to clients
- Maintaining accurate and up-to-date client records and reporting any changes or concerns to relevant parties
- Participating in continuing education and training opportunities to maintain knowledge and skills.

**Job Type**: Casual

**Salary**: $28.00 - $38.00 per hour
