As a Disability Support Worker at Empowering Connection, you will be an integral part of our mission to enable the creation of meaningful connections. Your responsibilities will encompass:

- Providing hands-on support tailored to each client's unique needs, encompassing personal care, mobility aid, and hygiene assistance.
- Crafting and implementing personalized care strategies that reflect individual goals and aspirations.
- Assisting clients with daily household tasks, meal preparation, and the management of medications.
- Organising and participating in engaging social and recreational activities to foster a sense of belonging and camaraderie.
- Observing, documenting, and communicating changes in clients' well-being, behavior, and progress.
- Collaborating closely with healthcare professionals, families, and caregivers to ensure holistic care plans.
- Upholding clients' rights and dignity, while promoting an environment that respects and nurtures their individuality.
