**The Opportunity**

The primary function of the Aftercare Case Manager is to support the most vulnerable care leavers aged 18 to 24 to achieve a successful transition to adulthood through case management/coordination and supported access to universal services. As a Case Manager with Samaritans, you will assist in developing leaving care case plans for young people in the Permanency Support Program and PSP Supported Independent Living Program, develop the financial plan for young people exiting care, be the main point of contact for care leavers wanting to access their Aftercare financial plans, and assist young people to access relevant services.

**The Essentials**
- Tertiary qualification in Social Work, Community Welfare, Psychology, Education or any other relevant tertiary qualifications and relevant transferrable experience
- Demonstrated knowledge of and capacity to work within relevant Aftercare and PSP guidelines, workplace policies and procedures and standards.
- Excellent understanding of the needs and complexities of the most vulnerable young people within the statutory system and those who have exited from the system at 18.
- Demonstrated ability to use a holistic based approach in implementing and overseeing case management and case plans
- Strong interpersonal skills that provide the basis for effective negotiation, mediation and conflict resolution practices and outcomes
- Sound professional knowledge and theory base particularly in the areas of trauma, attachment, and therapeutic care
- Highly developed case management documentation skills
- Proven ability to develop rapport with carers, young people and their families/significant others in order to support informed decision-making and skills development for living within the community
- Demonstrated commitment and ability to maintain confidentiality and accountability
- The ability to effectively develop and maintain positive working relationships and networks with other agencies
- Demonstrated ability to participate positively in teamwork, training and supervision processes
- Demonstrated understanding of the relevant Government legislation applicable to the Permanency Program
- Willingness to work outside of business hours.

The successful applicant will possess or willingness to obtain the following: NSW Working with Children Check, NDIS Worker Screening Check, first aid & CPR certificate and undertake a pre-employment functional medical.
