Come and join our team in Adelaide where we are currently recruiting for a Full-time Support Coordinator on a 6-month fixed term.

Lifestyle Solutions provide innovative and flexible person-centred support coordination services for people living with a disability. Support Coordinators assist our diverse clients to become more independent in their own homes and in the community.

Your responsibilities will include:

- Actively coordinate, monitor and report on supports in accordance with clients plans
- Actively support clients to meet their individual goals by identifying and accessing necessary services & supports
- Identify and respond to crisis situations, abuse & neglect, and critical incidents to promote a person's safety
- Maintain knowledge of safeguarding standards and reporting requirements
- Actively promote inclusion & uphold a clients human rights
- Engage/facilitate group practice reflection and decision making
- Participate in team-based goal setting and monitoring, (service delivery, financial viability, continuous & quality improvement)
- Service planning reviews and reporting to reflect internal and external client deliverables and satisfaction
- Support effective communication and partnering with identified stakeholders (i.e.: service providers, mainstream services, funding agencies, government departments)

**About You**:

- Current Driver’s License (mandatory requirement)
- NDIS Worker Screening Clearance (or be willing to obtain)
- Formal qualification in Social Sciences, Disability, Community Services, Psychology, Allied Health and/or equivalent experience (minimum Cert IV)
