**The** Community Care Services Department**

The Community Services Directorate comprises Aged Care Assessment Services, Community Allied Health Services, Community Care Services, Carer Support Services, and Community Nursing Services.

Community Care Services conducts a high-quality care coordinator service from 6 offices across the Loddon Mallee Region. Care Coordinator supports people who are frail aged, and younger people with a disability to remain living as independently as possible with the community.

The service manages:

- Home Care Package Program
- Short Term Restorative Care
- National Disability Insurance Scheme (NDIS)Support coordination
- Home and Community Care Program for Younger People (HACC PYP) assessment and home care
- Home and Community Care Program for Younger People (HACC PYP) Linkages

**Your new division**

The Community and Public Health Services Division has a primary focus on improving the health outcomes of the communities in our region. With a range of local and regional programs supporting place-based health promotion, prevention, care and support, they work collaboratively with other Divisions and regional partnerships and networks to support quality outcomes. The Division has a strong leadership role in the Loddon Mallee Region and is committed to ensuring processes are in place to deliver on our Division.

**Your new team**

Community Care Services conducts a high-quality care coordinator service from six offices across the Loddon Mallee Region. Care Coordinator supports people who are frail aged, and younger people with a disability to remain living as independently as possible with the community. As this is a secondment postion it is a great opportunity for an up and coming leader within the organisation to work within a very established team with a future focus on Reform changes that are coming into effect as of July 2025.
