Geelong- On-site- Full-timeThe Wellbeing Coordinator is responsible for managing the day-to-day service provision to Cultura’s home-based clients. Each Cultura Wellbeing Coordinator has an allocated caseload, but may also be required to contribute to overall consumer management. Duties include, but are not limited to, client assessment, community liaison, answering daily queries and problem solving with consumers/families. The aim of this position is to empower the consumer and ensure that they are re-engaged, encouraged to manage their own self-care and ensure that the consumer’s strengths are optimised.
- This position will be contracted on a permanent full-time basis, working 38 hours per week. _

**About You**:

- Work as a leader in the team that assesses, plans, provides and evaluates high quality, individualised home-based care for Cultura’s Home Care consumers.
- Develop user friendly and client focused practices and care partnerships that encourage individual Home Care consumers to self-determine and maximise their level of independence.
- Ensure home-based consumers are assessed when services are commenced, regular on-going consumer assessments are undertaken, and care plans maintained for individual Home Care consumers and that these reflect the actual care provided. Assessments must include a review of consumer’s actual ability and collaborative agreement of assessed needs.
- Ensure that documentation of consumer assessments and care plans is clear, and that all relevant input from medical and allied health professionals eg, doctor, podiatrist, physiotherapist etc. seen by a Home Care consumer is documented.
- Case manage Cultura’s Home Care consumers so that care and services are individualised to each client, ensuring all regular reviews and care changes are affected.
- Participate in program budgeting, as required, to ensure at least 80% of package funds are spent.
- Hold at least Certificate IV in Community Services, with a preference for a Diploma.
- Have recent experience managing/delivering direct care/home-based care services to elderly individuals.
