**Our Journey**:
The Social Work Department is looking for an experienced Grade 4 Social Worker to join our growing team at Monash Health, with a focus on clinical practice and innovation in the aged and rehabilitation setting and associated theoretical frameworks that guide practice. You will have the opportunity to join a dynamic social work and multi disciplinary team where you will have a pivotal role in contributing to a positive team culture.

**A Day in the Life**:
As a Grade 4 Social Worker you will be provided with an opportunity to be challenged plus demonstrate your skills including:

- Providing clinical consultation and leadership to your team
- Opportunities to develop your leadership skills in the area of clinical practice education, training and high level secondary consultation in collaboration with the Social Work leadership team

You will be a member of a dynamic and supportive Social Work and Allied Health team working in a collaborative model of care across Monash Health. The position works closely with the internal and external stakeholders to provide care founded on current evidence-based practice for patients accessing care through the inpatient and community based setting.

**What you will Bring**:
You will have demonstrated experience as a social worker and leader who is able to work collaboratively with the broader multidisciplinary team and possess excellent written and verbal communication skills. You will also require:

- Bachelor Social Work or Master Social Work (Qualifying)
- Experience working in a community health or hospital setting
- Current driver’s licence
- Current Working with Children’s Check
- Eligible for AASW membership including continuing professional education
- Post Graduate Qualifications (desirable)
