As a Disability Support Worker, you will play a crucial role in delivering services to young people we support in our Seven Hills Day Program. You will be tasked with enabling them to participate in their community, achieve their goals and live their best life. Achieve has been providing services to people with disability since the 1950s. Today, we are a registered NDIS provider offering integrated clinical, health, social and disability services in accommodation, community participation, supported employment and support coordination services in the Greater Sydney and in Northern Rivers regions in NSW.
**About the role**
- Promote the delivery of supports in accordance with Individual Support Plans to assist the client to remain safe and healthy.
- Provide front line staff with advice, coaching, training and feedback on general client care
- Support team members to implement Individual Support Plan and to gather and analyse information to identify issues and determine if changes are required to meet clients individual needs.
- Record and report identified change requirements to inform amendments to Individual Support Plans tailored to client’s individual needs.
- Complete mandatory documentation and any supporting documentation accurately and within given timeframes to meet operational requirements.
- Provide and promote timely reporting on incidents, risks and complaints in accordance with Achieve Australia policies and procedures and legislative requirements, and participate in any investigations as required.
- Monitor the implementation of behaviour support plans and provide client facing staff with support on positive behaviour support strategies, restrictive practices management and active support.
- Contribute to building and maintaining strong internal and external stakeholder relationships, including families and carers.
- Contribute to a culture of continuous improvement, active support and client safeguarding by demonstrating and encouraging practices to protect clients, children and other vulnerable people from harm and neglect.
- Participate in staff briefings and undertake training to ensure compliance with the organisation’s policies, processes and procedures, to meet the NDIS Practice Standards, Quality Indicators and all regulatory and legislative requirements.
**About you**
- Demonstrated ability to take initiative and work independently in providing support to individuals with complex needs, including a track record of proactive problem-solving.
- Proven resilience in high-pressure situations, particularly when dealing with physical and verbal aggression, showcasing an ability to maintain composure and provide effective support during challenging moments.
- Substantial experience working with individuals with high support needs, complex health requirements, and restrictive practices, highlighting the ability to navigate and implement comprehensive support strategies.
- Strong communication skills, particularly in adapting communication methods for individuals with impaired communication, ensuring effective understanding and responsiveness to the unique needs of the person being supported.
- Consistent and reliable work history, with a proven ability to meet responsibilities and commitments, including adherence to schedules and protocols essential for providing continuous care.
