Memorable Care Pty Ltd is seeking a sociable, organised, and dedicated Support Coordinator to join our team. In this role, you will support NDIS participants and their families in understanding and implementing their funded supports and connecting them with the appropriate services. We are looking for someone who is ready to go the extra mile, ensuring our participants receive the highest quality of care and support.

Key Responsibilities:

- Provide Support Coordination services with a focus on participants from CALD backgrounds, helping them navigate the NDIS to achieve their goals.
- Work collaboratively with participants, families, and service providers to develop and implement individual plans.
- Facilitate multi-disciplinary and coordinated care planning while building strong relationships with service providers.
- Maintain up-to-date knowledge of NDIS legislation, price guides, and policies.
- Engage with NDIA staff, Local Area Coordinators (LACs), and other stakeholders.
- Prepare for NDIS plan reviews and attend review meetings alongside participants and their families.
- Ensure accurate, timely, and confidential record-keeping, reporting, and RIDS monthly reporting.

Qualifications & Experience:

- Demonstrated experience in Support Coordination, Recovery Coaching, LAC or a similar role (minimum 2 years).
- Preferably from a medical background who has deep knowledge on mental health disabilities and some physical disabilities as most of our participants have combined disabilities or genetic disabilities that require complex supports.
- Degree level qualification in a relevant field social worker, psychology, allied health, community services or mental health as we are gearing for SSC.
- Fluent in languages other than English and who have in-depth knowledge of specific cultural communities as 95% of our participants are from diverse backgrounds.
- Has deep, established relationships with service providers in the local community or a particular sector (e.g., healthcare, therapy, housing) to access specialized services more quickly and negotiate tailored support packages.
- Exceptional written and verbal communication skills, with the ability to engage positively and collaborate effectively with diverse stakeholders.
- Ability to manage crises and ensure continuity of care in challenging situations with participants who have complex or high-risk needs. This includes individuals with dual diagnoses (e.g., mental health issues combined with intellectual disabilities), those experiencing homelessness, or people with high support needs.
- Comprehensive understanding of NDIS legislation, rules, and pricing structures.
- Ability to work independently and collaboratively as part of a team.
- Strong time management, organizational, and administrative skills.
- Strong written and verbal communication skills, with the ability to prepare professional reports and maintain accurate records.
- Ability to maintain a high level of confidentiality and handle sensitive information appropriately.
- Results driven, solutions focused.
- Experience working with individuals with a justice background (preferred).
- Knowledge in navigating the intricacies of funding, eligibility, and plan management to secure the best outcomes for their participants.
- Familiarity with facilitating multi-disciplinary planning and building collaborative relationships.
